Home Health Registered Nurse – Full Time

JOB SUMMARY:
The Home Health Care RN position is the cornerstone of VNA of Somerset Hills’ clinical operations. RNs provide nursing care for those with illness and/or injury wherever the patient calls home. This care is achieved through care plan development and oversight including clinical nursing interventions, education, and supervision of care plan implementation for patients and caregivers. Our RNs also provide consultation and referral to appropriate personnel and services within the agency and the community.


PRIMARY QUALIFICATIONS:

  • Comprehensive knowledge and skill in current nursing practice and an understanding and knowledge of the nursing process
  • Knowledge or interest in home care philosophy and practice
  • Completion of a baccalaureate degree in nursing from a college or university accredited by the National League for Nursing is preferred OR graduation from an accredited school of nursing
  • Current license to practice as a Registered Nurse in the State of New Jersey or has license pending as indicated by a permission-to-work letter from the New Jersey State Board of Nursing

  • Knowledge of disease process, causative factors, and value of physical therapy agents in its relief.
  • Knowledge of safety and infection control procedures.
  • Knowledge of Medicare and third party payor rules/regulations for home care.
  • Basic knowledge of the OASIS form, OASIS function and OASIS scoring.
  • Basic computer skills.

DUTIES AND RESPONSIBILITIES:

  • Attends and completes the orientation program provided by the Agency.
  • Maintain standards as set by the APTA regarding treatment procedures, professional ethics and standards of practice.
  • Administer physical therapy services as per M.D. orders:
    a. Responds to physicians for confirmation, clarification or elaboration of treatment orders.
    b. Recommends/implements changes in treatment plan with MD approval and signature; based upon the patient’s response to therapy.
  • Provide services in the categories of:
    a. Admission Visit (Start of Care) including patient sign-off for consent of visits.
    b. Physical therapy evaluation and home safety assessment.
    c. Patient care planning/establishment of treatment goals.
    d. Implementation of treatment program to relieve pain, develop or restore function, and maintain maximal performance.
    e. Provides appropriate discharge planning and refers to appropriate resources if indicated.
    f. Provides patient/family education to include a relevant home of home program with instruction to patient, family, and assistive personnel.
    g. Utilize therapeutic modalities that are available for the home setting.
    h. Instruct patient/family/caregiver in the use of assistive devices: wheelchair, walker, canes, crutches, prosthetic/orthotic devices and Hoyer lift.
    i. Direct and assist patient in therapeutic exercise, muscle re-education, gait, transfers and functional training, ADL’s and prosthetic training.
    j. In service training to patients/family/caregivers and the community at large.
  • Provide referrals to ancillary support disciplines (MSW, Registered Dieticians, etc.)
  • Participate in staff meetings and attend mandatory in service educational programs.
  • Maintain adequate records and dcoument treatment per Medicare/Payor guidelines.
  • Responsible for OASIS scoring accuracy and ongoing OASIS education
  • Follow proper safety and infection control procedures.
  • Coordinate with patient/family to schedule physical therapy visits; keeping them informed of any changes that may occur. Keep therapist rescheduling to a minimum..
  • Therapists are not permitted to make any arrangements with a patient where you will provide services on a private basis. The individuals for whom you provide service are clients of the Agency, and options do exist for private payment to the Agency.
  • Therapists will coordinate time off with the Rehab Manager and/or Director of Clinical Services in order to insure adequate coverage for the therapy service.
  • Demonstrate flexibility and willingness to travel to the various geographical locations of our patients, and help cover patients for vacationing therapists.
  • Works weekends on rotation to cover orthopedic protocols for join-replacement patients and/or other designated patients. If unable to cover accepted weekend rotation due to illness or emergency, the therapist will find their own replacement.
  • Perform duties within the established policies of the VNA of Somerset Hills and all associated regulatory agents/agencies such as CHAP, OIG, OSHA, State, etc.
  • Accept and perform other duties as assigned.

On Call Nurse – Per Diem

The VNA of Somerset Hills has a long and respected tradition of delivering personalized, compassionate, and community-focused healthcare. JOB SUMMARY: As an On Call Nurse, you will be responsible for responding to patient/family phone calls and making visits as indicated during off hours of the weekday and weekend (4:30 pm to 8:30am Monday thru Thursday; Friday 4:30pm to Monday 8:30am). You would respond to the needs of Hospice patients, patients with catheters and other patients requiring 24-hour availability. The On Call Nurse may also need to make unplanned but medically necessary skilled visits to established patients when needed, or unplanned admissions.

QUALIFICATIONS:

  • Completion of a baccalaureate degree in nursing from a college or university accredited by the National League for Nursing is preferred or Graduation from an accredited school of nursing
  • Current license to practice as a Registered Nurse in the State of New Jersey or has license pending as indicated by a permission to work letter from the New Jersey State Board of Nursing
  • Current New Jersey Drivers’ License
  • A minimum of two years’ experience in Home Care or Hospice Nursing
  • I.V. certification is required

  • Phlebotomy certification is preferred

DUTIES & RESPONSIBILITIES:

  • Promote and maintain health of individuals, families and the community through teaching, counseling and appropriate preventative and rehabilitative measures
  • Demonstrate, teach and supervise care to patients and families
  • Respond to all calls within a maximum timeframe of 15 minutes
  • Participate in On-Call Holiday coverage and weekend coverage
  • Consult about individual or family health problems with the Administrative Call Nurse, and other professionals, such as physicians, dieticians, social workers and therapists
  • Promote continuity of patient care by planning and exchanging information with physicians and other health and social agencies
  • Participate on Hospice Team and in other team meetings, including presentation of cases, as needed

Hospice Registered Nurse – Per Diem

Job Summary:
This is a great opportunity for an experienced and committed hospice RN to join a leading central NJ hospice care provider. In this position you will manage and coordinate patient care with a multidisciplinary team of hospice professionals.

Qualifications:

  • A registered nurse with a current NJ state license
  • BSN preferred
  • Minimum 2 years hospice experience required
  • CHPN preferred

LPN/RN -Referral Service Center Liaison

Job Summary:  We invite you to apply for employment at our highly regarded independent community healthcare agency. The VNA of Somerset Hills has a Referral Service Center Liaison LPN/RN opening with an attractive compensation and benefits package. We are seeking New Jersey licensed nurses who have an interest in customer relations, and coordination of services.

Responsible for accurately and expediently entering preliminary patient referral information into system and ensuring that Referral Services Center calls are answered in a timely manner.  This team member demonstrates competence in multi-tasking and prioritizing work, which ensures that we exceed our internal and external customer expectations.  Also responsible for working with the discharge planning team at hospitals and health care facilities to facilitate and expedite appropriate home care placements with the agency so patients may be appropriately discharged from the facility and receive home care services as expeditiously as possible after discharge.

Reports to: Associate Director of Referral Services and Sales 

Qualifications:

   Required:

  • Current license to practice as a Registered Nurse or Licensed Practical Nurse in the State of New Jersey
  • Excellence in communication/organization skills.
  • Computer proficiency in Outlook, Word and internet

    Preferred:       

  • Bachelor’s Degree preferred
  • Previous referral and/or intake experience
  • Health care marketing experience
  • Knowledge of Home Care Home Base

 

Duties and Responsibilities:

  • Interprets agency policy to patients and facility personnel.
  • Answer calls to Referral Service Center, consults with physicians, nurses and other disciplines to obtain referral information and provides feedback to facility personnel.
  • Enters all preliminary patient referral information into computerized “point of care” system as needed. Inputs/processes referrals into the system in a timely and accurate manner
  • Communicates with internal departments and external customers regarding initial patient visits to ensure positive customer satisfaction.
  • Participates in quality improvement activities.
  • Builds professional relationships with patients, families, providers, internal customers to ensure service excellence.
  • Demonstrates knowledge of computer systems, electronic health record, scheduling system and other technology utilized within the organization.
  • Assist in evaluation of patients referred by facility staff for home care to determine appropriateness for admission to VNASH programs. Works with discharge planning team at facilities to refer patients needing other sources of care or appropriate service.
  • Consults with physicians, nurses and other disciplines while establishing a coordinated home care plan prior to discharge.
  • Provides feedback to facility personnel, as needed and/or requested.
  • Evaluate Hospice referrals for eligibility. Coordinate needed service for Hospice admission. Work with pharmacy to arrange medication delivery. Work with DME supplier to arrange equipment delivery.
  • Process Hospice referral to include obtaining of MD order, referral entry, scheduling of initial visit, process of Hospice CTI and worksheets for worksheets for MD completion
  • Serves as a liaison between facilities and VNASH.
  • Collects and maintains statistical data of referred patients, as needed.
  • Collaborates with VNASH Referral Service Center regarding patient plan of care and needs.
  • Serves as a resource to facility staff on community resources and home care issues and best practices.
  • Carries out the mission, vision and pillars of the organization.
  • Other duties as assigned.                       
Certified Home Health Aides (CHHA) – Per Diem and/or Shift Work

JOB SUMMARY: 
To provide direct personal care to the sick and/or disabled assigned by any of the VNA of Somerset Hills programs, with full regard for the dignity of each of those patients. Direct personal care may include personal hygiene, activities of daily living and the care of the environment directly related to the safety and well being of the patient and insuring the patient’s optimum level of self care.

REPORTS TO: Clinical Manager

QUALIFICATIONS:

  • Successful completion of a home health aide certification training program approved by the State of NJ Board of Nursing.
  • Current home health aide certification
  • Current NJ driver’s license along with a safe driving record.
  • High school diploma or the equivalent preferred.

KNOWLEDGE & ABILITIES:

  • Ability to work well with people.
  • Ability to carry out the physical tasks required by the job.
  • Ability to communicate both orally and in writing in English.
  • Ability to read maps and follow directions.
  • Reliability e.g. following the assigned schedule consistently.
  • Dependability e.g. being at assignment at correct time and submitting paper work accurately and in a timely manner.

DUTIES & RESPONSIBILITIES:

Certified Home Health Aides may perform the following duties under the direction and supervision of the Home Health Aide Supervisor or the Nurse Case Manager of a particular case which the CHHA is assigned.

  • Assist patient with bath.
  • Assist patient with toileting needs.
  • Assist patient in and out of bed, assisting with ambulation and exercise.
  • Assist patient with self-administered medications.
  • Perform incidental household services.
  • Accompany patient to a doctor’s appointment, hospital or clinic for treatment by means of approved mode of transportation. (if on Private Care Services)
