The Visiting Nurse of Somerset Hills is dedicated to promoting health and wellness in our communities.  In addition to our home health programs, we have a number of high-quality, cost-effective community health programs available to the residents of Somerset and Morris counties.

Flu Immunization Clinics

Flu shots are available at no charge to people who have Medicare Part B and for a $25 fee to non-Medicare patients. Flu shots are available on a seasonal basis. Click here for flu clinic schedule.

Blood Pressure Screenings

Blood pressure screenings are available monthly to seniors in specific communities. Click here for schedule.

Immunizations

Along with regular checkups, immunizations are the best way to keep all children healthy. Immunizations and well-baby check-ups are available to all uninsured infants and children from birth through kindergarten who live in: Bedminster, Bernards Township, Bernardsville, Chester Borough, Far Hills, Long Hill Township, Mendham Borough, Mendham Township, & Peapack-Gladstone. Parish Nursing Program

Through our Parish Nursing Program, volunteer nurses trained by a VNA Parish Nurse Coordinator, assist individuals in maintaining basic health and wellness while meeting the unique needs of their congregation. Click here for more information.

In Home Caregiver Education

Through the In Home Caregiver Education Program caregivers receive one-on-one education on maintaining a safe environment for their ailing loved-one while assuring preparedness as an illness progresses. Click here for more information.

Geriatric Care Management

Our Geriatric Care Management program is designed to help the elderly and their families with the challenges of aging and planning for long-term care. A nurse provides an in-depth assessment of the older person's needs, and together with the family, develops a plan of care that addresses their needs. Click here for more information.

Friendly Visitor Program

Our Friendly Visitor Program brings trained volunteers to the homebound elderly and the disabled. Click here for more information.

Companion Program

The Companion Program is a non-medical basic support service designed to meet the needs of those persons who need monitoring or socialization. Click here for more information.

FARE

FARE (Friends and Relatives of the Elderly) is a forum to provide peer support among friends and relatives who care for the elderly and chronically disabled. It also provides educational information on the process of aging and the resources available for caregivers. Click here for locations and times.
 
Respite Care
The Respite Care Program offers Somerset County families short-term relief from the everyday fatigue and stress of caring for a frail elder or disabled adult. Click here for more information.

Medication Management
A Registered Nurse is available to assist individuals manage their medications on an ongoing basis. For more information about this service and the cost please contact our Intake Manager at (908) 766-0180, EXT 141.

Local Public Health Partnerships

The Visiting Nurse Association of Somerset Hills works with the Bernards Township Department of Health in tracking, monitoring and following-up on reports of communicable diseases in our communities.  In addition, we are an active participant in Somerset and Morris Counties' Bio-Terrorism planning activities.
