The VNA of Somerset Hills is committed to helping individuals stay in their home and community for as long as possible but we realize that for some elderly, every day can be challenge. We can help guide you through the maze of challenges and assist you in finding help through our Geriatric Care Management Program.

What is geriatric care management?

The Visiting Nurse Association of Somerset Hills Geriatric Care Management program will send a registered nurse or licensed social worker experienced in working with older adults to your home. The geriatric care manager will assess the individual's needs and make recommendations regarding home safety, support services, community resources, transportation, housing options and/or long term planning.
