

About Telehealth:
Telehealth is a technologically advanced system that enables specially trained VNA nurses to assist frail, older adults in managing their illness. A Telehealth visit is like having a home visit over the television. Using existing telephone lines, this PC-based interactive videophone provides for two-way monitoring of qualified patients in their own home.
Telehealth serves:
Patients with a primary diagnosis of congestive heart failure and/or other chronic illnesses who are at high risk for hospitalization. These patients initially require in-home visiting nurse services and are homebound.
Telehealth Services:
- A monitoring unit that has the capacity to check heart and lung sounds, blood pressure, blood oxygen level and blood sugar
- A telehealth nurse for counseling and education and assessment of the health of the patient
- Live, on-screen, two-way monitoring with visual access to monitor overall patient care or remote monitoring provided by a RN with experts in cardiac care.
The Benefits of Telehealth:
Telehealth augments the nurses’ visits to the home and engages high-risk patients in necessary self-care. These additional patient contacts allow for earlier detection of changes in health status and intervention, thereby decreasing and/or preventing re-hospitalization. Also, for many isolated patients, it provides an enhanced sense of well-being.
