Home Health Services

The Visiting Nurse Association of Somerset Hills has been providing home and community health care services to the residents of Somerset and Morris Counties, New Jersey for over 100 years. Experts in managing a patient’s care in the home, after a stay at the hospital or when recovering from an illness or injury, the VNA has the knowledge and capability to provide a broad range of services.


Skilled Nursing

Working in conjunction with a physician, a Registered Nurse will act as case manager of the patient’s home care. The nurse will come to the house to evaluate a patient’s needs and develop a plan of care.  Through regularly scheduled visits to the home, the nurse, along with other members of a health care team, will attend to the patient’s clinical needs, provide teaching and support and coordinate delivery of additional services, as needed.   

Social Worker

A licensed Social Worker will asses the needs of patients and families and present them with emotional support strategies and referrals to appropriate community services.

Certified Home Health Aides

In conjunction with skilled nursing care, home health aides are available to provide personal care, meal preparation and assistance with daily living activities. Our Home Health Aides are certified through the New Jersey Board of Nursing and are supervised by the RN on all cases.

Rehabilitation Therapies

  • Physical Therapy – A licensed Physical Therapist will conduct a comprehensive evaluation of the functional level and, in conjunction with a physician, will develop a plan of treatment. The treatment will be carried out through a schedule of regular in-home visits.
  • Occupational Therapy – A licensed Occupational Therapist, in conjunction other members of the home care team, will develop and implement an in-home treatment plan to retrain and rehabilitate essential life skills lost as a result of an illness or injury.
  • Speech Therapy – A licensed Speech Language Pathologist, working in conjunction with other members of the home care team, will assess the patient’s needs and develop a plan of in-home care that will improve communication and/or speech, swallowing and language difficulties occurring as a result of illness or disability.

Registered Dietician

A Registered Dietician will evaluate the patient’s nutritional needs, develop a diet plan and teach them how to successfully follow special diet recommendations. 

IV Therapy

Infusion services are available to patients who need IV Therapy in the home.

Telehealth Montoring

Using today’s telecommunications technology, a Registered Nurse remotely visits a patient in the home to check heart and lung sounds, blood pressure, blood oxygen, sugar levels and other vital signs. For more detailed information on Telehealth Monitoring click here.

Wound Care Management

A nationally certified Wound Care Nurse uses state of the art procedures and products in the care of wounds, helping to reduce healing time and complications.

Home Safety Assessments

Home safety assessments will point out modifications and repairs that can be done to one’s house that can reduce or prevent an accident. These assessments are available form the VNA on a private pay basis and in some cases may also be covered by Medicare, if you or a loved one is homebound.

We also offer Hospice and Palliative Care in the home to residents of Somerset and Morris county in New Jersey.
