Hospice Services


CHOICE. COMFORT. SUPPORT. PEACE. 

Hospice is an option.

Hospice is a special way of caring for the terminally ill and their families in the home. When a loved one is dying, our hospice program enables the patient to remain in the comfort of his or her own home with family close by. The Somerset Hills Hospice is a Medicare-certified and state-licensed hospice, which focuses on comfort, support and dignity by providing a wide range of services to the patient and family in the home setting, who have chosen this end of life option. Hospice services can also be provided for a terminally ill patient living in a nursing or assisted living facility.

Because only terminally ill patients are accepted in the Hospice Program, the family and friends often need assistance in coping with their loss. Supportive counseling is available during Hospice and through 13 months following death of the patient. Click here for more information on bereavement counseling.

Hospice services include:

  • Skilled Nursing
  • Social Work
  • Certified Home Aide
  • Trained Volunteers
  • Rehabilitation Therapies: Physical, Occupational and Speech, as needed
  • Clergy
  • Bereavement Support Group
  • Registered Dietician

The Hospice Team:

  • Somerset Hills Hospice Team includes physicians, nurses, social worker, home health aides, pharmacist, pastoral counselor and volunteers. The team works closely with the patient’s primary physician to coordinate care.
  • The Somerset Hills Hospice Team has developed specific expertise in the management of pain, and the relief of symptoms. The team administers to the patient's needs and counsels the family on social, financial and emotional issues. 
  • Other specialists are available as needed, such as home health aides, physical, speech and occupational therapists, a pharmacist and dietitian.
  • Arrangements can be made for supplies and equipment such as wheelchairs, hospital beds and walkers.

The Goal of the Hospice Program:

To provide for all care needs at the patient’s place of residence in the comfort of familiar surroundings whenever possible.

  • To help the patient live as fully and comfortably as possible.
  • To assist the family in the care of the patient.
  • To continue support even if the patient is transferred to a hospital or nursing home.
  • To provide continuing support to the family during the grieving process.

Eligibility:

  • Patients with a terminal illness, living in Somerset or Morris County or eastern Hunterdon County, and consenting to receive Somerset Hills Hospice services.
  • The patient must have a primary caregiver who is responsible for the care between professional visits.

Payment for Hospice Care:

  • Services are paid through Medicare, Medicaid, managed care and other medical insurances, with sliding scale fees available. No patient will be refused for lack of funds. Somerset Hills Hospice is supported by the Somerset County United Way.
  • In addition, the Sondra Geier Hospice Fund accepts donations and memorial gifts from private individuals, local service organizations and foundations to help patients and families receive care when financial resources are limited.
