About Palliative Care:

Palliative Care means comfort care – emphasizing quality of life while facing a life-threatening illness.  Palliative Care focuses on the comprehensive management of the physical, psychological and spiritual needs of patients with progressive, incurable illnesses and bridges the period of time when a patient wishes the expertise of home care for comfort and symptom control but is not yet ready for hospice care.
 
Eligibility:

Patients who have been diagnosed with a life-threatening illness are eligible for Palliative Care.
 
The Palliative Care Team:

Palliative Care is provided by a core interdisciplinary team consisting of the patient’s physician, nurse, medical social worker, and palliative care coordinator in the comfort of the patient’s home. These core services may be supplemented with the support of home health aides, dietician and trained volunteers.  Complementary therapies may also be offered.  
 
Services Provided:

The following services are provided by our Palliative Care program:

• Pain and symptom control
• Skilled nursing care
• 24 hour telephone availability of Hospice/Palliative Care Nurse
• Social work / psychosocial counseling
• Volunteer assistance
• Bereavement support group

Payment for Palliative Care:

Payment for Palliative Care is available through third party insurance or grant funding.
