To refer a patient to the Visiting Nurse Association of Somerset Hills for health care services please call our Intake Manager directly, during our regular business hours, Monday through Friday 8:30 AM – 4:30 PM at 908-766-0180, EXT 141.

Physicians may also download our referral form, complete it and fax it to our Intake Manger at 908-766-6534. Click here for referral form.

Our Intake Department is available to answer any questions regarding our services during regular business hours at the number listed above. For questions and to submit referrals after regular business hours or on weekends call 908-766-0180 and ask to speak to the administrator on call.
