Medical Social Worker (MSW) Morris County – Per Diem

JOB SUMMARY: Provides social work service to home health patients and their families as well as consultation to staff members. Acts as a resource person in matters pertaining to complex personal, social, emotional and environmental difficulties which cause social and personal maladjustment or which predispose toward illness or interfere with obtaining maximum benefit from health care.

REPORTS TO: Manager, Social Work


  • Completion of a masters degree program in social work (MSW) at an accredited graduate school of social work
  • Current license to practice Social Work in the State of New Jersey (LSW or LCSW)
  • Current New Jersey Drivers License with insurance proof


  • A minimum of one year post master’s experience in a health care setting with a demonstrated comprehensive knowledge of social work skills


  • Knowledge of VNA mission, philosophy, policies and programs
  • Ability to adapt skills to the home health setting
  • Knowledge of VNA Home Care, Palliative Care and Hospice philosophy, policies and programs
  • Knowledge of community resources
  • Effective communication skills; both written and oral
  • Computer literate
  • Ability to establish and maintain constructive working relationships with staff and clients/families
  • Ability to maintain patient confidentiality
  • Ability to utilize supervision in a constructive way


  • Upon referral of a patient the social worker will:
  • a. Assess, in a timely manner according to VNA protocol, patient and family
    functioning in relation to health care needs including financial, environmental social and emotional needs.
    b. Develop and assist in the implementation of a plan of care, which may include counseling, emotional support, coordination of and referral to community resources, and discharge planning
    c. Provide social work services to the patient’s family member or caregiver on a short term basis when it can be established that such an intervention will aid in the patient’s recovery
    d. Participate in interdisciplinary case conferences for the development of treatment plans and interpretation of significant social, emotional and environmental factors relating to the patient’s health problems
    e. Consult with staff members to further develop family assessment and counseling strategies
    f. Report to the physician changes or factors in the patient’s psychosocial condition as appropriate
    g. Maintain accurate and timely records and appropriate patient communication as per agency policy

  • Develop and maintain professional relationship with other service providers in the community as appropriate
  • Identify gaps in service delivery and assist in planning, developing and implementing programs to meet these needs within the agency and the community
  • Assist Home Care team members to understand the significant social and emotional factors related to the care of the patient and the family
  • Keep abreast of trends and developments in the field of Social Work and Home Care by attending continuing education programs
  • Carry out other duties as assigned
