Working in conjunction with a physician, a registered nurse will act as case manager of the patient’s home care. The nurse will come to the house to evaluate a patient’s needs and develop a plan of care. Through regularly scheduled visits to the home, the nurse, along with other members of a health care team, will attend to the patient’s clinical needs, provide teaching and support and coordinate delivery of additional services, as needed.
Hospice is a special way of caring for the terminally ill and their families in the home. When a loved one is dying, our hospice program enables the patient to remain in the comfort of his or her own home with family close by. Hospice services can also be provided for a terminally ill patient living in a nursing or assisted living facility.
