Through our in-home Telehealth Monitoring Program, individuals with serious illnesses such as diabetes, chronic obstructive pulmonary disease (COPD), heart failure, high blood pressure and coronary artery disease remain living safely and independently at home. We focus on helping our patients learn to better self-manage their chronic conditions and enjoy an improved quality of life.
A specially-trained VNA registered nurse develops a customized care plan in conjunction with the patient’s physician that includes nursing consultations and the use of technologically advanced equipment. The telehealth monitor records daily vital signs including blood pressure, heart rate, blood oxygen, weight and blood glucose levels.This information is then remotely monitored by the nurse located at VNA headquarters.
Patient education and counseling are an important part of this program. Our nurses help patients understand their condition, how to manage medications and the importance of lifestyle factors such as weight, exercise, nutrition and diet.
