Telehealth Nurse – Per Diem

JOB SUMMARY: Provides, arranges for, teaches, and supervises nursing care of the sick and injured in any of the VNA Programs, through the use of telemonitoring, when it is appropriate. Telemonitoring, or a “telecontact”, may be done via a non-video unit in the patient’s home, with a Telehealth Nurse reviewing the patient-entered data.

REPORTS TO: Chronic Care Manager

QUALIFICATIONS:

  • Completion of a baccalaureate degree in nursing from a college or university accredited by the National League for Nursing is preferred.
    or Graduation from an accredited school of nursing
  • Current license to practice as a Registered Nurse in the State of New
    Jersey or has license pending as indicated by a permission-to-worklLetter from the New Jersey State Board of Nursing
  • Current New Jersey Drivers License
  • Basic computer skills

EXPERIENCE:

  • A minimum of two years experience in nursing. Experience in Cardiac Nursing preferred.

KNOWLEDGE & ABILITIES:

  • Knowledge of VNA mission, vision, philosophy, policies and programs
  • Comprehensive knowledge and skill in current nursing practice
  • Knowledge of Hospice philosophy and practice
  • Knowledge of Medicare Conditions of Participation for Home Health Services
  • Basic knowledge of computer systems and their functions
  • Ability to organize, manage and function in assigned areas
  • Ability to accept and utilize supervision
  • Ability to work collaboratively with members of the Management Team
  • Ability to recognize needs and concern for people
  • Ability to establish and maintain constructive working relationships with individuals and families

DUTIES & RESPONSIBILITIES:

  • Assess patients for appropriateness of Telehealth monitoring, making a
    “Telehealth Assessment” home visit if needed
  • Schedule and make a home visit within one week of patient acceptance to program to set up Telehealth monitor and instruct the patient/caregiver in the operation and care of this equipment
  • Make home visit, if needed, for technical problems, replacement of parts or retrieval of equipment from the home. Clean /replace equipment after retrieval from home when volunteer(s) not available
  • Schedule and make all Telehealth contacts
  • Promote and maintain the health of individuals through teaching, counseling and appropriate preventive and corrective measures via Telehealth monitoring
  • Record observations and other significant information. Maintain adequate records at all times
  • Consult about individual and family health problems with Nurse Case Manager, Nursing Supervisor and other professionals, such as physicians, dieticians, social workers and therapists as appropriate
  • Promote continuity of patient care by planning and exchanging information with (other) Telehealth nurses, Nurse Case Manager, physicians, other health and social agencies as appropriate
  • Help the patient to accept appropriate medical, hospital, nursing home, social service assistance or other care as necessary. Interpret extent and limitations of service available
  • Ensure the completion of the Telehealth Patient Satisfaction Survey at appropriate intervals
  • Consult/update Chronic Care Manager, Director of Quality Care Management and/or Director of Professional Services about Telehealth technical problems; maintain adequate communication with Chronic Care Manager(s) and Director of Quality Care Management about current status of Telehealth patients and the program in general
  • Collaborate with the Director of Professional Services, Nurse Managers, Referral Service Manager, Director of Quality Care Management, Director of Development & Marketing and President & CEO to expand the program to a larger and varied patient population as appropriate as well as evaluate and improve the Telehealth Program
  • Serve as Nurse Case Manager for VNA Community Health Services Telehealth-only patients
  • Carry out policies of the Visiting Nurse Association of Somerset Hills
  • Carry out other duties as assigned
